As a key member of our team, you'll work collaboratively with colleagues to provide a supportive environment, respecting individuals' preferences and choices, and enabling their fullest participation in community life.

Key Responsibilities:

* Support individuals in all aspects of their daily life, enabling their fullest participation in community life
* Support people to achieve their personal outcomes
* Liaise with external professionals and devise support plans and risk assessments
* Communicate specialist knowledge to colleagues to produce a coherent and integrated package of support
