We are seeking an exceptional Palliative Care Specialist Nurse to join our team at Worcestershire Acute Hospitals NHS Trust. As a key member of our hospital-based specialist palliative care team, you will work closely with the CNS team to develop your clinical skills in end-of-life and specialist palliative care delivery.
Key Responsibilities- Provide specialist and holistic palliative care assessments of patients and their carers' needs, minimizing physical symptoms and the social, financial, emotional, and spiritual implications of the disease process.
- Formulate a plan of care using specialist knowledge and skills, dependent on the level of specialist intervention required, in negotiation with the patient, their carers, and the wider multidisciplinary team.
- Provide specialist care and advice for patients and carers, and assist the multidisciplinary team in delivering and evaluating interventions/ plans of care, reviewing and amending as appropriate with support from the CNS.
- Act as a primary resource for site-specific specialist nurses, nursing/medical teams, and the multidisciplinary team, facilitating and assisting with complex discharge planning to home, hospice, or other facilities.
- Initiate and participate in highly sensitive and complex communication issues with patients, carers, nursing/medical teams, including pre-bereavement support, advice, and imparting life-changing news.
- Recognized NMC teaching and assessing qualification
- Non-medical Prescribing qualification
- Advanced Communication Skills training
- Significant post-registration experience at Band 5
- Experience of working within the field of palliative care
