We are recruiting for the position of Adult Support Specialist, which involves working as part of our Hospital Discharge team. As a key member of our team, you will collaborate with healthcare professionals, therapists, and nurses to ensure seamless transitions for patients from hospital to community care.

Responsibilities

As an Adult Support Specialist, your primary responsibilities will include:

  • Conducting comprehensive assessments to identify individual needs and preferences;">
  • Developing personalized care plans that prioritize patient choice and control;">
  • Collaborating with multidisciplinary teams to ensure holistic approaches to patient care;">
  • Maintaining accurate records and reports to inform decision-making;">
  • Providing guidance and support to colleagues less experienced in social care processes;">
  • Staying up-to-date with relevant legislation, policy, and frameworks governing adult social care.">

Requirements

To be successful in this role, you will possess:

  • ">
  • A CQSW/CSS/Diploma/Degree in Social Work or equivalent qualification;">
  • Registration with Social Work England;">
  • Able to undergo an enhanced DBS check and Adults Barred List check;">
  • Excellent communication and interpersonal skills;">
  • Ability to work effectively in a fast-paced environment;">
  • Familiarity with relevant legislation, policies, and frameworks governing adult social care.">
