Job Description:

**Job Summary**

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting individuals with complex needs, disabilities, and long-term conditions to live independently in their own homes.

**Key Responsibilities**

* Provide 1:1 support to individuals with Paranoid Schizophrenia and Learning Disabilities, promoting their independence and well-being
* Develop and implement personalized care plans, taking into account the individual's needs, preferences, and goals
* Work collaboratively with a mixed-gender team to provide 24-hour support, including day and night shifts
* Implement detailed care plans and risk assessments to ensure the individual's safety and well-being
* Provide encouragement and support to individuals with complex health needs and Learning Disabilities
* Implement PBS (Positive Behavioral Support) approaches to promote least restrictive practice

**Requirements**

* Social care experience, ideally within mental health and with some experience of supporting individuals with Schizophrenia
* Experience providing encouragement to individuals with complex health needs and Learning Disabilities
* Experience supporting individuals with behaviors that may be considered challenging
* Current Right to Work in the UK and at least 6 months of employment in the UK
* Enhanced DBS check and satisfactory references
