About the Role:
We are seeking skilled professionals to join our Llanelli and Ammanford team as key workers. In this role, you will assess the needs of individuals recently discharged from hospital or requiring urgent care, creating person-centred care plans and risk assessing properties. You will work closely with external agencies to ensure service users receive necessary support.
