Falkirk Council is seeking a dynamic and enthusiastic Senior Home First Practitioner to join the Home First Team. This is a new role within the team, offering an exciting opportunity for team development and sustainable change within the service.
Key Responsibilities:
- Provide leadership and management to the Home First Team, ensuring the delivery of high-quality services to clients.
- Develop and implement effective strategies to improve service outcomes and client satisfaction.
- Collaborate with local authority services, data protection teams, and other stakeholders to ensure compliance with legislative requirements.
- Supervise and mentor team members, providing guidance and support to ensure their professional development.
- Contribute to a culture of ongoing improvement and a busy service, demonstrating a positive 'can do' attitude and drive to 'make it happen'.
Requirements:
- Education to SVQ Level 3 in social care, CSS, Diploma in Social Work, or equivalent.
- Excellent numeracy and literacy skills, with the ability to work with data and IT systems.
- Knowledge and experience of local authority services, data protection legislative requirements, and the Home First ethos.
- Experience within a health and/or social care setting, with excellent leadership and management skills.
- Excellent communication, supervision, and IT skills, with the ability to work to deadlines within a busy team.
What We Offer:
- A 37-hour post, based between Falkirk Community Hospital and Forth Valley Royal Hospital, depending on demand.
