About the Role:
We are seeking an experienced and dedicated professional to join our Hospital Discharge team at Central Bedfordshire Council. As a key member of our team, you will play a vital role in supporting individuals to achieve their goals and maximize their independence.
Key Responsibilities:
- Conduct home visits and assessments to identify individual needs and develop personalized support plans.
- Work closely with healthcare professionals, social care teams, and other stakeholders to ensure seamless transitions and effective discharge planning.
- Provide emotional support and guidance to individuals and their families, promoting confidence and empowerment.
- Collaborate with the reablement team to ensure a smooth transition from hospital to home, focusing on rehabilitation and reintegration into the community.
- Manage a caseload, prioritizing tasks and ensuring timely interventions to meet individual needs.
Requirements:
- Relevant qualifications in social work, nursing, or occupational therapy, with current registration with the relevant professional body.
- Proven experience in discharge coordination, community support, or a related field, with a strong understanding of the care act and mental capacity act.
- Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
- Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
- Access to a vehicle and a full, valid driving license.
