About the Role:

We are seeking an experienced and dedicated professional to join our Hospital Discharge team at Central Bedfordshire Council. As a key member of our team, you will play a vital role in supporting individuals to achieve their goals and maximize their independence.

Key Responsibilities:

  • Conduct home visits and assessments to identify individual needs and develop personalized support plans.
  • Work closely with healthcare professionals, social care teams, and other stakeholders to ensure seamless transitions and effective discharge planning.
  • Provide emotional support and guidance to individuals and their families, promoting confidence and empowerment.
  • Collaborate with the reablement team to ensure a smooth transition from hospital to home, focusing on rehabilitation and reintegration into the community.
  • Manage a caseload, prioritizing tasks and ensuring timely interventions to meet individual needs.

Requirements:

  • Relevant qualifications in social work, nursing, or occupational therapy, with current registration with the relevant professional body.
  • Proven experience in discharge coordination, community support, or a related field, with a strong understanding of the care act and mental capacity act.
  • Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Access to a vehicle and a full, valid driving license.
