At Voyage Care Ltd, we are seeking a highly skilled and dedicated Care Management Specialist to lead our residential service in Chard/Somerset. This is an exciting opportunity for someone who is passionate about enhancing the quality of life of the people we support.

The successful candidate will have previous experience as a care management professional, with a proven track record of delivering high-quality care and support plans. They will be responsible for managing the day-to-day running of the service(s), developing and implementing person-centred support plans, conducting care needs assessments, recruiting and inducting new colleagues, and ensuring regulatory and legislative requirements are met.

We offer a competitive salary of £39,624 per annum, depending on experience. In addition to this, we provide a range of benefits, including:
