Job Title: Complex Needs Practitioner Level 2

Key Responsibilities

  • To work collaboratively with colleagues to develop and implement personalized support plans.
  • To maintain accurate records and contribute to quality assurance activities.
Requirements

Candidates must have relevant experience in a similar role, preferably within the care or social work sector. A qualification in social care or a related field is also desirable.
