* Develop and implement effective rehabilitation and discharge plans
* Provide education and training to patients, families, and healthcare staff

**Requirements:**

* Strong clinical skills in assessing and treating a range of conditions
* Ability to develop and implement effective rehabilitation and discharge plans
* Excellent communication and teamwork abilities
* Demonstrates evidence of recent CPD and an ability to reflect on experiences
* Experience with community-based pathways is advantageous
