Job Summary: Spalding Primary Care Network seeks a Clinical Care Specialist to join their team. This role will be part of a multi-disciplinary team within Spalding PCN, working to deliver health services as outlined in the Network Contract DES Specification PCN Requirements and Entitlements.
The successful candidate will have key responsibilities in assessing and triaging patients, providing definitive treatment or making necessary referrals. They will also advise patients on general healthcare and promote self-management. This role will be an integral part of the general practice team, working as and when required in each organisation and across Spalding PCN.
Key Responsibilities:
- Work as part of a multi-disciplinary team (MDT) within Spalding PCN.
- Assess and triage patients, providing definitive treatment or making necessary referrals.
- Advise patients on general healthcare and promote self-management.
- Perform specialist health checks and reviews within their scope of practice.
- Interpret ECGs and perform investigatory procedures as required.
- Support the delivery of anticipatory care plans.
- Provide an alternative model to urgent and same-day GP home visits.
- Communicate effectively with all colleagues across the MDT.
- Maintain accurate health records and documentation.
- Communicate complex and sensitive information effectively with patients and their families.

Requirements:
- Willingness to work flexible hours.
