About the Role

The successful candidate will be required to work autonomously, identifying, assessing, and treating patients, while also contributing to the development of clinical pathways and protocols.

Key Responsibilities
  • Assess and diagnose patients with frailty needs, developing and implementing individualized care plans
  • Provide clinical leadership and support to the multidisciplinary team, promoting best practice and quality improvement
  • Contribute to the development of clinical protocols and guidelines, ensuring evidence-based practice
  • Collaborate with other healthcare professionals to ensure seamless care transitions and effective discharge planning
Requirements
  • Qualified nurse or Allied Health Professional with Advanced Practice degree
  • Significant clinical experience in community assessment and frailty care
  • Excellent communication and interpersonal skills, with the ability to motivate and lead others
  • Ability to work autonomously, prioritizing tasks and managing time effectively
