The successful candidate will act in an enabling capacity, helping to organise appointments and correspondence, and motivating and supporting the client in accessing leisure, health, and social services as part of an individual programme plan.

Key Responsibilities:

  • Provide support and enablement to the client in daily activities
  • Assist with organisation of appointments and correspondence
  • Support the client in accessing leisure, health, and social services
  • Work collaboratively with the client and other professionals to develop and implement an individual programme plan

Requirements:

  • Enhanced DBS check required
  • Flexibility to work across weekdays and weekends
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
