About the Role:
We are looking for a Mental Health Discharge Specialist to join our team at Devon Partnership NHS Trust. This is a 12-month fixed-term position working 30 hours per week for a Discharge Facilitator based at Haytor ward.
The successful candidate will have experience of multi-disciplinary/multi-agency working, particularly regarding housing, accommodation, and support providers. They will also have knowledge of the Care Act, Mental Health Act, and Mental Capacity Act.
This role offers a competitive salary of £35,000 - £45,000 per annum, depending on experience. In addition, we offer a range of benefits including flexible working arrangements, a comprehensive training program, and opportunities for career development.
The successful candidate will be responsible for developing and accurately recording comprehensive support plans, risk assessments, and relevant reports to inform care management and social care processes.
