About the RoleAs a Registered Nurse - Children's Continuing Care (Community), you will be responsible for delivering high-quality care to children and young people with complex health needs in their own homes and community settings. You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate care plans that meet the individual needs of each child or young person. You will also be responsible for providing education and support to families and carers to enable them to provide the best possible care for their child or young person.
Key Responsibilities- Assess and monitor the health and wellbeing of children and young people with complex health needs
- Develop and implement individualized care plans that meet the unique needs of each child or young person
- Provide education and support to families and carers to enable them to provide the best possible care for their child or young person
- Work collaboratively with a multidisciplinary team to ensure seamless care delivery
- Contribute to the development and implementation of policies and procedures that promote high-quality care and service delivery
- Registered Children's Nurse with a valid NMC registration
- Minimum of 6 months paid experience in a Registered Nurse Children's Continuing Care (Community) environment within the last 2 years
- Valid UK driving license and access to a car
- Ability to work flexible hours, including evenings and weekends
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
