Care Planning Specialist

">About the Role:

We are seeking an experienced Adult Social Worker to join our team in Pembrokeshire. This permanent position involves assessing user needs, developing care packages, and contributing to budget management.

Key Responsibilities:
  • User Needs Assessment: Assess user strengths and needs using established formats.
  • Care Package Development: Develop and cost care packages that meet individual needs.
  • Budget Management: Manage adult care budgets and maintain expenditure within limits.
  • Care Planning: Undertake specialist social work care planning and direct interventions.
  • Review and Coordination: Review and coordinate care package reviews to ensure effective service delivery.
  • Training and Development: Participate in training opportunities to enhance professional skills.
Requirements:
  • Social Work Qualification: Possess a Social Work qualification or equivalent.
  • Continual Professional Development: Demonstrate a commitment to continual professional development.
  • Social Work England Registration: Be registered with Social Work England.
  • Adult Social Work Experience: Have relevant experience working in adult social work.
Estimated Salary:

£32,000 - £42,000 per annum depending on experience.
