About the Role

An exciting opportunity has become available to work in Urgent Community Response, supporting admission avoidance and hospital discharge patients.

The role involves multidisciplinary working across acute and community services to ensure patients are supported into the right pathway in a timely manner.

The team consists of Nurses, Occupational Therapists, Rehab Support Workers, Support Workers, and trainee Nursing Associates.

Working Pattern

Hours per week across 7 days from 8am - 8pm.
