The role involves multidisciplinary working across acute and community services to ensure patients are supported into the right pathway in a timely manner.
The team consists of Nurses, Occupational Therapists, Rehab Support Workers, Support Workers, and trainee Nursing Associates.
Working PatternHours per week across 7 days from 8am - 8pm.
