About the Role

We are seeking a highly motivated and compassionate Support Worker to join our Integrated Outreach Team. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with psychosis and complex needs.

Key Responsibilities
  • Work as a member of the Integrated Outreach Team to provide support and care to individuals with psychosis and complex needs.
  • Develop and maintain relationships with service users, their families, and other stakeholders to ensure comprehensive care and support.
  • Contribute to the development and implementation of individual care plans, ensuring that service users' needs are met and their goals are achieved.
  • Collaborate with other healthcare professionals to ensure seamless care and support, and to promote the recovery needs of service users.
  • Provide education and support to service users and their families on mental health, well-being, and recovery.
Requirements
  • Experience of working in a care or support role, preferably in a community care setting.
  • Understanding of the role of the Integrated Outreach Team and the needs of individuals with psychosis and complex needs.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and other stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of service users.
  • Knowledge of the benefit system and ability to navigate the housing application system.
