Job Title: Empower Family Group Supported Living Support Worker
Job Summary:
We are seeking a highly motivated and enthusiastic Supported Living Support Worker to join our team at Empower Family Group. As a Supported Living Support Worker, you will play a vital role in supporting young adults aged 16+ in their transition to independent living.
Main Responsibilities:
- Support young people in a range of practical, personal, social, and financial ways to enable and develop their independence.
- Complete weekly and monthly reports, support plans, and risk assessments to ensure accurate recording and planning.
- Identify risks and concerns, and liaise with the wider team to put plans in place to ensure young people's safety.
- Support young people to re-join or continue with their education and learning journey or take the first steps on their journey to employment.
- Ensure the placement appearance is kept to a high standard, including daily cleaning and maintenance.
- Help young people to enjoy affordable and sustainable activities within their local community.
- Use creative and inclusive approaches to support young people's development and improve their aspirations through 1-1 key work sessions.
- Liaise with external agencies and professionals.
- Adhere to policies and procedures to safeguard young people from harm and abuse.
- Manage challenging behavior and difficult situations.
Requirements:
- Full UK driving license.
- Right to work in the UK.
- GCSE or equivalent.
- NVQ Level 3 qualification in Health & Social Care (preferred).
