We are seeking a highly motivated Clinical Lead to join our newly established Learning Disability and Autism (LDA) Outreach Team at Devon Partnership NHS Trust. This exciting opportunity involves working collaboratively with colleagues from various backgrounds, including Mental Health and Learning Disability nursing, AHPs, Occupational Therapy, and Speech and Language Therapy.

The ideal candidate will have extensive clinical knowledge, advanced leadership skills, and excellent safeguarding, Mental Health Act, and Mental Capacity Act knowledge. They will also possess advanced interpersonal, communication, formulation, and risk assessment skills.

The main duties of this role include triaging, assessing, planning, and delivering complex care packages to admission-vulnerable service users across Devon. The successful applicant will be able to work autonomously and as part of a multiprofessional team, ensuring proactive liaison with other partner agencies, families, and care providers at every stage of care.
