An exciting opportunity has arisen for a highly motivated and skilled healthcare professional to join our Single Point of Access (SPA) team at Oxford Health NHS Foundation Trust.
The successful candidate will have the ability to work autonomously, using their advanced triaging skills to prevent avoidable hospital admissions and provide high-quality patient care in a community setting.
We are looking for an excellent communicator who can work effectively within a team, demonstrating a commitment to delivering positive outcomes for patients in the community.
Main Duties:
- To deliver and maintain a high standard of triage, using enhanced assessment, problem-solving, and decision-making skills to identify patient needs and appropriate service destinations.
- To manage clinical risk and make referrals to various community services, including urgent response, crisis, social care, community hospitals, and specialist services.
- To work closely with experienced clinicians and administrators, receiving training and support to ensure effective job performance.
