EHCP Coordinator

Role Overview:
We are partnering with a progressive Primary School in Swindon dedicated to providing exceptional support to children with diverse and complex needs. As the EHCP & Referral Coordinator, you will play a crucial role in managing Education, Health, and Care Plans (EHCPs) and coordinating referrals to ensure students receive the tailored support they need.
Key Responsibilities:
- Collaborating with parents, carers, and professionals to gather information about a child's SEND
- Conducting assessments and reviews to determine the child's needs
- Facilitating multi-disciplinary meetings to develop EHCPs
- Acting as the central contact for all EHCP-related communications
- Coordinating assessments, reviews, and meetings for timely information exchange
- Maintaining accurate records and documentation
- Liaising with educational professionals to implement EHCP provisions
- Monitoring and reviewing the child's progress, ensuring EHCP provisions are met
- Participating in annual reviews and evaluating EHCP effectiveness
- Providing reports and updates to Local Authorities and relevant bodies
About You:
- Be able to do the job as described
- Familiarity with the EHCP assessment and planning process
- Proficient in assessing the needs of children with SEND
- Excellent communication and interpersonal skills
- Strong organisational and time management skills
- Ability to collaborate effectively with various professionals and agencies
