Job Summary:
Sanctuary Personnel Ltd is seeking a highly skilled and dedicated Adult Social Worker to join our team in a permanent position within the Community Support Services. This role will be based in Brent Cross and will involve working full-time for Jewish Care.
Key Responsibilities:
- Collaborative Care Provision: Work collaboratively with individuals, carers, and relevant resources to provide personalized care, reducing risk and improving quality of life.
- Multi-Disciplinary Approaches: Actively participate in collaborative and multi-disciplinary approaches, building networks within and external to Jewish Care.
- Community Interventions: Support the development of community and group interventions to enhance social and community life.
- Forum Participation: Contribute to forums focusing on Dementia, Disability, Family Carers, Mental Health, Safeguarding, Mental Capacity, End of Life Care, and Holocaust Survivors.
- Service Development: Work with the Team Manager and Senior Social Workers to develop and run an effective service.
- Relationship Building: Develop relationships within Jewish Care, Statutory, and Voluntary organizations.
- Service Representation: Present the work of the Social Work and Community Support Service in various forums.
- Information Provision: Provide information and advice following current government health and social care guidance.
- Qualifications and Experience: A professional Social Work qualification or equivalent, and experience working with diverse populations facing Mental Health, Mental Capacity, Disability, Dementia, Isolation, End of Life, and other co-morbidities.
