About the Role

As a Community Nurse Case Manager, you will conduct initial assessments in the community, providing high-quality care to clients with complex needs. You will work closely with our client, a leading complex care provider, to deliver exceptional care and support.

Key Responsibilities
  • Conduct initial assessments in the community to identify client needs and develop personalized care plans.
  • Provide high-quality care and support to clients with complex needs, including peg feeds, ventilators, and tracheostomy management.
  • Collaborate with our client's team to ensure seamless care delivery and excellent client outcomes.
Requirements
  • Be an RGN or RSCN Qualified Nurse with excellent clinical skills in complex care.
  • Have experience in tracheostomy and complex care.
  • Be a car driver, flexible to travel around the North West.
  • Have experience caring for both adults and children.
