Key Responsibilities:

  • Provide comprehensive medical care to patients served by Hospital at Home.
  • Conduct thorough patient assessments, including vital signs, medical history, and current symptoms.
  • Develop and implement tailored care plans based on patient needs and medical conditions.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Collaborate with healthcare professionals to ensure coordinated and effective care.
  • Document patient progress and maintain accurate records.

Requirements:

  • Qualified Clinical Associate (Clin A), Registered Nurse (RN) or Enrolled Nurse (EN) with up-to-date SANC/HPCSA registration.
  • Prior experience in a hospital or home healthcare setting is preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Empathy and compassion for patients and their families.
