About the Role

We are seeking an exceptional individual to join our team as a Cancer Care Navigator at Sandwell and West Birmingham Hospital NHS Trust. This pioneering role is part of our Living with and Beyond Cancer Program, and we are looking for someone who is passionate about improving the experience of cancer patients and their carers.

Key Responsibilities
  • Work closely with Cancer Services and our Cancer Nurse Specialist Team to provide support and guidance to cancer patients and their families.
  • Assist in the smooth running of nurse-led clinics, MDT Outcomes, and patient tracking.
  • Deliver Holistic Needs Assessments and Care Planning telephone clinics to support patients in resolving non-complex needs and concerns.
  • Provide reassurance, telephone patient triaging, and support, alongside patient-related information giving and signposting.
Requirements
  • GCSE English language and Mathematics (Grades A-C) or equivalent.
  • NVQ 3 plus additional training.
  • Recognised ECDL or equivalent or extensive experience using Microsoft office package.
  • Experience of working in the NHS in a patient-facing role.
  • Experience of multi-professional working.
  • Experience in the use of data management.
  • Experience of coach/teaching patients and carers.
