Key Responsibilities
  • Conduct thorough medication reviews, including face-to-face, remote, and e-consultations, to ensure safe and effective medication use.
  • Reconcile medications following hospital discharge and collaborate with community pharmacists to ensure seamless care transitions.
  • Support long-term condition reviews through face-to-face and telephone appointments to optimize patient care.
  • Participate in regular safety audits to identify and mitigate potential risks associated with medication use.
  • Maintain accurate and detailed records of patient interactions, medication changes, and other relevant information.

In addition to these responsibilities, the successful candidate will be expected to maintain their professional competence through ongoing education and training.
