Care Team Leader

We are seeking a compassionate and dedicated individual to join our team as a Team Leader. As a key member of our care team, you will be responsible for supporting the home management team in delivering high-quality care with a difference.
Your focus will be on providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. You will lead in the person-centred care planning process, assist in the smooth transition of discharges, and monitor the well-being of each resident.
Key Responsibilities:
- Lead in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations.
- Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed.
- Monitor the well-being of each resident, being aware of any change impacting care and care planning, and ensuring that changing needs are met and written records are amended to reflect any variation.
About You
To succeed in this role, you will have an NVQ 3 or equivalent in care for care homes, two years' experience working in a senior position with the resident group, and the ability to communicate effectively both verbally and in writing.
