About the Role:

  • Deliver high-quality care and support to clients, promoting independence, dignity, and well-being.
  • Work closely with healthcare professionals to develop and implement individualised care plans.
  • Contribute to a supportive and collaborative team environment, sharing knowledge and expertise to improve client outcomes.
  • Develop and maintain relationships with clients, their families, and healthcare professionals.

Requirements:

  • Able to work flexible hours, including evenings and weekends.
  • Valid driving licence (desirable).
  • Enhanced DBS check required.
