Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced Registered Nurse to fill the role of Band 6 Triage Nurse. This exciting opportunity allows for a comprehensive telephone triage and clinical assessment service to patients and their carers/relatives. The successful candidate will be working autonomously, providing a safe and appropriate clinical decision. Estimated salary range: £43,041 - £70,305.

Main Responsibilities

">

The clinical triage role is based within the Planned Community Nurse Team, supporting the clinical triage of new patient referrals and messages. These new referrals come through Single Point of Access (SPA) from various services, including GP/Hospital/Health Care Professionals/self-referral/social workers. The post holder will assess these referrals against the IMT specification to determine if the patient meets the criteria for a Community Nurse Team visit. Additionally, the role involves triaging messages from patients on the current caseload to establish if a visit is needed and the urgency of the visit. Effective time management, organisational/prioritisation skills, and ability to meet deadlines are essential for this position.

Person Specification

">

To succeed in this role, you must possess a District Nurse Specialist Practitioner Qualification or be willing to work towards equivalent modules. Sound knowledge and application of up to date evidence-based clinical practice, excellent communication analytical skills, and problem-solving approach are also required. A competent user of MS Word, Excel, Access, PowerPoint, and Patient Administration Systems is necessary for effective task completion.
