Job Summary

The Elizabeth Glaser Pediatric AIDS Foundation is seeking a highly skilled and experienced Linkage Nurse to join our team. The successful candidate will be responsible for leading the organization and establishment of linkage systems at the facility and beyond, collaborating with MOH clinic staff to identify gaps in service delivery, and implementing adolescent PSS, Facility AGYW, DREAMS activities, and differentiated service delivery models.

Key Responsibilities
  • Lead the implementation of linkage systems, ensuring seamless transitions between care settings and services.
  • Collaborate with MOH clinic staff to identify gaps in service delivery and develop strategies to address them.
  • Implement adolescent PSS, Facility AGYW, DREAMS activities, and differentiated service delivery models to improve health outcomes.
  • Provide direct service delivery in MNCH, Pediatric, and adult ART clinics, focusing on indicator performance.
  • Ensure eligible clients access Viral Load Testing and provide quality Intensive adherence counselling and follow-up.
  • Strengthen, scale-up, and track the performance of mother-infant pair (MIP) clinics as a differentiated service delivery model.
  • Work with MOH staff to ensure all mothers and children accessing services in MCH have a known HIV status and promote HTS to those with unknown HIV status.
  • Support and strengthen integration of PMTCT and EIDT in MNCH.
  • Work with MOH staff to strengthen the provision of Family Planning services in all ART delivery points.
  • Support MOH and other partners implementing community programs on the linkage of all HIV clients with community structures like support groups.
  • Promote retention strategies through implementation of appointment Registers and early tracking of missed appointments.
  • Liaise with HDAs to ensure viral load coverage, facilitate screening of master cards, and follow-ups of detectable VL.
  • Support Covid-19 screening and prevention practices within the ART clinic and collaborate with MOH to facilitate Covid-19 vaccination among PLHIV, at-risk populations, and community.
  • Assess NCDs (Diabetes, Hypertension, Depression, and Substance Abuse) among PLHIV and plan for management in liaison with MOH, Site clinical coordinator, or District Clinical Mentor.
  • Support ASOs in case management by ensuring correct stratification of recipients of care, follow-up, and outcome documentation.
  • Support CQI initiatives by playing a leading role in follow-up of action points and collaborating with the CQI lead on rapid learning cycles to ensure achievement of targets.
  • Support MOH or Site Clinical Coordinator in management of Advanced HIV Disease by strengthening screening of eligible recipients of care, facilitating lab tests, and documentation of results.
Requirements
  • Diploma or Degree in nursing and midwifery.
  • More than 5 years of experience in HIV/TB management, with training in current ART management and registration with relevant Registration bodies.
