Key Responsibilities- Embed and provide personalised care through all of our services, ensuring a patient-centred approach.
- Empower patients to lead the lives they want to live by understanding their needs and preferences.
- Develop and maintain effective relationships with patients, families, and carers to deliver tailored care.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and effective use of skills and experience.
- Contribute to the development and implementation of new ways of working and delivering care, prioritising patient needs and outcomes.
To be successful in this role, you will need to possess:
- Strong communication and interpersonal skills.
