Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at Harrogate and District NHS Foundation Trust. As a Community Nurse, you will be responsible for providing high-quality nursing care to individuals with complex needs in the comfort of their own homes.
Job OverviewAs a member of our community team, you will work closely with a range of professionals to deliver excellence in care. We pride ourselves on being proud to be part of the Harrogate and Rural Alliance, and we are committed to providing innovative and high-quality care to our patients.
Main Responsibilities- To undertake the provision of high standards of nursing care in relation to the Integrated Care Team
- To plan, implement and evaluate episodes of care.
- To plan and organise workload and delegate to other staff when necessary.
- To undertake nursing assessment, diagnosis and treatment appropriately.
- To undertake generic assessments on behalf of the team
- Registered Nurse with current NMC registration
- Diploma/Degree in Nursing or related field
- Post registration clinical experience
- Ability to drive and access to transport in order to meet service needs is essential
