About the Role

We are seeking an experienced Mental Health Community Nurse to join our Crisis Function team. As a key member of the team, you will work with service users at various stages of their mental health journey, providing urgent assessments and robust home treatment options as an alternative to hospital admission.

Main Responsibilities
  • Provide high-quality, person-centered care to service users, ensuring their needs are met and their care plans are updated regularly.
  • Work collaboratively with the multi-disciplinary team to provide safe and effective treatment in the community, facilitating early discharges from acute inpatient units.
  • Develop and implement care plans, working closely with service users, their carers, and colleagues to positively manage risk and enable recovery.
  • Deliver evidence-based care, adhering to Trust policies and procedures, and demonstrating a high standard of record keeping and documentation.
  • Communicate effectively with service users, their families, carers, and professionals, both verbally and in writing, at an age-appropriate level.
