Job Title: SCPHN Health Visitor

Job Summary: We are seeking an experienced and motivated Occupational Therapist to join our Rehabilitation and Frailty Team (RAFT) in North East London. As a key member of our team, you will work closely with physiotherapists, occupational therapists, nurses, rehabilitation assistants, and integrated healthcare assistants to provide high-quality patient-centered care.

Main Responsibilities:

  • Deliver daily therapy sessions in patients' own homes, prioritizing caseloads to respond to changing needs and communicating closely with other service providers.
  • Work collaboratively with the team to maintain independence, promote self-care, and assist patients in achieving their goals.
  • Participate in the development of the RAFT service, contributing to the creation of a friendly and approachable team environment.

Requirements:

  • Experience working at B5/B6 level and previous NHS work.
  • Degree in Occupational Therapy and HCPC registration.
  • Knowledge of OT processes, patient-centered care, and clear communication skills.

Working for NELFT:

NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and undertake a comprehensive induction, including mandatory training, systems training, and equipment allocation.

Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period unless they are currently part-way through a probationary period or are a bank member of staff.

High Cost Area Supplement: This post attracts payment for a High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,258 and a maximum payment of £2,122 per annum pro rata).
