About the Role

We are seeking an experienced Physiotherapist to join our Early Supported Discharge Team, where you will play a vital role in providing high-quality stroke rehabilitation services in patients' own homes and community settings. This challenging yet rewarding position offers a unique opportunity to work closely with interdisciplinary teams and make a significant difference in patients' lives.

Key Responsibilities
  • Manage a caseload of clients being discharged home from hospital early, coordinating discharge and providing high-quality stroke-specific assessment and implementation of treatment plans.
  • To be responsible for the planning and delivery of high-quality stroke-specific physiotherapy within the Early Supported Discharge Team and other community stroke services as required.
  • Work effectively and flexibly within an interdisciplinary framework to provide integrated stroke rehabilitation care programmes for patients on the community stroke pathway within ESD (and CICC as required) for rehabilitation assistants to carry out.

Requirements

To succeed in this role, you will need:

  • BSc Hons or recognised qualification in Physiotherapy
  • Holds valid UK driving licence and access to a car
  • Able to demonstrate efficient and effective time management, organisation, planning, and problem-solving skills
