About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Oaklands Health Centre. This is a challenging and rewarding opportunity to work in a dynamic environment, providing high-quality patient care.

Job Summary

The successful candidate will be responsible for delivering minor illness clinics, providing triage services, and prescribing medication as necessary. They will also support the clinical team in developing individual treatment plans for chronic disease patients and identify patients at risk of developing long-term conditions.

Main Responsibilities:
  1. Health Promotion and Well-being: Develop and implement health promotion and well-being programs to support the delivery of patient-centered care.
  2. Triage Services: Provide a triage service, actively managing or referring patients or treating patients within own scope of practice.
  3. Maintenance of Accurate Records: Maintain accurate clinical records in conjunction with extant legislation.
  4. Requesting Pathology Services: Request pathology services as necessary, processing and interpreting results as required.
  5. Patient Support: Recognize, assess, and refer patients presenting with mental health needs, develop strategies for effective care pathways, and liaise with external services/agencies to ensure the patient is supported appropriately.
  6. Delegation of Clinical Responsibilities: Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
  7. Supporting the Clinical Team: Support the clinical team with all safeguarding matters, in accordance with local and national policies.
