About the Role

We are seeking a skilled Care Professional to join our team as a Home Care Assessor in Gloucestershire.

This is a field-based position, where you will travel throughout the county using one of our pool cars to visit clients and care workers. Your primary responsibility will be to complete initial client assessments, ensuring that their needs are met and they receive the best possible care.

Key Responsibilities
  • Conduct thorough assessments of clients' needs and develop personalized care plans
  • Visit clients and care workers in their homes or other settings, providing support and guidance as needed
  • Maintain accurate records of client interactions and assessments
Requirements

To be successful in this role, you will need:

  • A strong background in social care or a related field
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team
