Reablement Team

A short-term service aimed at empowering individuals to regain confidence and independence in their daily lives.

As a successful team, we have achieved an impressive 80% rate of service users maintaining their independence without formal support.

The Role

You will be responsible for:

  • Assessing and monitoring the needs of service users, ensuring they receive tailored care and support
  • Liaising with professionals and the wider team to contribute to the mental and physical health monitoring of service users
  • Assisting with daily tasks such as bill management, grocery shopping, and prescription collection, promoting connections with local community agencies
Requirements

To succeed in this role, you will need:

  • Proven experience in caring for vulnerable adults with complex needs
  • The ability to work independently and exercise initiative
  • A full driving license and access to a vehicle while working
  • Fluency in English, as required by Part 7 of the Immigration Act
