Pharmacist - Independent Prescriber Role

Job Summary
The Principal Medical Limited is seeking a highly skilled and experienced Pharmacist to join our team as a GP Practice Pharmacist with Independent Prescribing Qualification. This is an exciting opportunity for a motivated and dedicated individual to work in a dynamic and supportive environment, contributing to the delivery of high-quality patient care.
Main Responsibilities
The successful candidate will be responsible for providing clinical pharmacy services to patients in a general practice setting, working closely with GPs and other healthcare professionals to optimize patient care. Key responsibilities will include:
- Medication Review and Optimisation: to review and optimize patients' medication regimens, ensuring safe and effective use of medications.
- Medicines Reconciliation: to review and reconcile patients' medications upon admission or transfer, identifying potential medication errors or discrepancies.
- Medicines Information/Education: to provide patients and healthcare professionals with accurate and up-to-date information on medications, including their uses, side effects, and interactions.
- Prescribing Systems and Policies: to develop and implement prescribing policies and procedures, ensuring compliance with relevant guidelines and regulations.
- Chronic Disease Medicines Management: to provide specialized care for patients with chronic diseases, such as diabetes, hypertension, and asthma, working closely with GPs and other healthcare professionals to optimize patient care.
- Liaison with Community and Hospital Pharmacies: to communicate and collaborate with community and hospital pharmacies, ensuring seamless care transitions and optimal patient outcomes.
- Prescribing: to prescribe medications as necessary, working within their scope of practice and in accordance with relevant guidelines and regulations.
- Teaching/Training: to participate in the education and training of healthcare professionals, including medical students and GP trainees.
Requirements
To be successful in this role, the candidate will require:
- Mandatory registration with the General Pharmaceutical Council.
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
- Minimum of 2 years post-qualification experience in a similar role.
- Excellent interpersonal, influencing, and negotiating skills.
- Excellent written and verbal communication skills.
- Ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
- Good IT skills, including Microsoft Office, email, and internet software.
