**Job Summary:**

We are seeking a Community Care Provider to join our team in Leeds, UK. The successful candidate will be responsible for delivering high-quality care services to individuals in our community, promoting independence, and fostering positive relationships with service users and carers.

Key Responsibilities:

  • Be proactive in identifying and addressing any concerns or issues related to care delivery.\
  • Develop and maintain knowledge of local resources, services, and support networks.\
  • Collaborate with healthcare professionals and other agencies to ensure seamless care coordination.\
  • Keep accurate and up-to-date records of client interactions and progress.\
  • Review and revise care plans as necessary to reflect changing needs and circumstances.\
  • Ensure compliance with regulatory requirements and industry standards.\

Location: Leeds, UK

Shift Patterns: Monday-Friday 9:00am-3:00pm, alternate weekends 7:00am-2:00pm or 4:00pm-10:00pm.
