About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with mental health needs in the community.

Key Responsibilities
  • Assess and manage complex caseloads, including specialist assessments and care planning in conjunction with service users and carers.
  • Participate in the setting of quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Deliver a range of interventions, including pharmacological and psychologically informed interventions, in accordance with agreed plans of care and pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans, promoting positive risk-taking to improve quality of life and maximize occupational performance/independence.

  • Registered Mental Health Nurse on the NMC Register.
  • Conforms to NMC requirements.
  • Experience of working with people experiencing mental health-related difficulties and those who have difficulty engaging with services and have complex mental health needs.
