Job Description

We are seeking a skilled Nurse to join our team at Livewell Southwest. The successful candidate will work as a member of our DN Treatment Clinics/Wellbeing Lower Limb Service and will be required to work independently during their working day.

The post holder will demonstrate a history of interest in lower limb management and tissue viability. They will be responsible for providing specialist advice and information for the holistic assessment and management of lower limb disease.

Main Duties

  • Embed competency frameworks
  • Provide holistic evidence-based care to patients
  • Assess, plan, implement, and evaluate care packages
  • Collaborate with the clinical nurse specialist/team manager/team leaders
  • Problem-solve and liaise with the community team manager/team leaders
  • Involve in triaging referrals, preventative strategies, and maintenance of lower limb health
  • Maintain accurate records
  • Undertake comprehensive risk assessments
  • Monitor and maintain the health, safety, and security of self, others, and patients

Requirements

  • Current UK Level 1 registration with the NMC
  • Applications from newly qualified nurses are welcome
  • Full UK driving license
  • Experience of working within community/clinic settings, delivering complex nursing skills
  • Background and development knowledge of government policies and changes in primary care
