Job Description

As a Senior Specialist Nurse at University Hospital Southampton NHS Trust, you will have the opportunity to work as a core member of the multi-disciplinary team, providing specialist nursing for a defined patient/client group.

Key Responsibilities
  1. Practice Autonomously: You will be responsible for practicing autonomously in care settings and advising Divisions regarding the overall management of the service.
  2. Develop Seamless Care: You will work across professional, departmental, and Trust boundaries to develop and support patient-centred care, with particular responsibility for leading nurse-led care initiatives.
  3. Holistic Assessment and Planning: You will assess and identify the health and psychosocial needs of individual patients and plan a Personalised Care programme to meet these needs.
  4. Establish Contact and Provide Support: You will establish contact with patients/carers from the time of initial referral or identification and provide specialist nursing advice and practical and psychosocial support.
  5. Deliver Knowledgeable Nursing Care: You will deliver knowledgeable nursing care to patients in practice settings, working as part of nursing teams and in partnership with ward and departmental clinical leaders.
  6. Empower Patients: You will communicate with patients in ways that empower them through shared decision making to make informed choices about their health and healthcare.
  7. Provide Education and Support: You will provide appropriate education to patients and their families to promote health and encourage self-care and participation in the planned programme of treatment and care.
  8. Communicate Sensitively: You will use advanced communication skills to impart sensitive, complex, and potentially distressing information to patients and carers and provide them with advice and emotional support.
  9. Maintain Patient Documentation: You will maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting.
  10. Contribute to Clinical Activity: You will contribute to clinical activity/data collection as required.
  11. Work Collaboratively: You will work collaboratively and in partnership with other healthcare professionals, offering appropriate guidance and supervision to colleagues.
  12. Develop and Implement Standards: You will be involved in planning and implementing standards of care, practice guidelines, and Integrated Care Pathways and continually evaluate the quality of patient care.
  13. Contribute to Nurse-Led Initiatives: You will contribute to the development of nurse-led initiatives for the benefit of the identified client group in relevant care settings.
  14. Demonstrate and Disseminate Clinical Practice: You will demonstrate and disseminate clinical practice developments, utilising research in the specialist area of practice to develop and deliver evidence-based care.
  15. Support Change and Service Improvement: You will be proactive in supporting change to promote practice development and service improvement in accordance with local and national service policy or guidance.
  16. Participate in Delivering the Corporate Nursing Agenda: You will participate in delivering the University Hospital Southampton NHS Trust corporate nursing agenda.
  17. Challenge Professional Boundaries: You will challenge professional and organisational boundaries in the interest of patients and clients and to improve health outcomes.
  18. Develop Protocols and Guidelines: You will contribute to the development of protocols, documentation systems, standards, policies, and clinical guidelines for others to use in practice.
  19. Work with Health and Social Care Colleagues: You will work in collaboration with health and social care colleagues within the Trust and external primary and acute care providers to develop patient-centred pathways of care.
  20. Evaluate Service and Patient Care: You will assist in the evaluation of the service and patient care, including patient satisfaction.
  21. Provide Education and Training: You will provide education/training opportunities for nursing and other learners at pre and post-qualification levels through classroom teaching and clinical placements.
