We are seeking a Senior Practitioner to join our team as we continue to develop our transitions service. The successful candidate will be an important part of the leadership team, supporting the Team Managers to further develop the service and providing day-to-day consultation, direction, and support to the practitioners.

Key Responsibilities

  • Support the Team Managers to develop the transitions service
  • Provide day-to-day consultation, direction, and support to the practitioners
  • Carry a small caseload of challenging and complex cases
  • Lead by example to model best practice

Requirements

  • Qualified and registered Social Worker or Occupational Therapist
  • Minimum of 2 years post-qualifying experience for Social Workers
  • Previous experience working in the community or with young adults/transitions services for Occupational Therapists
  • Ability to travel across the City
