The successful candidate will be an important part of the leadership team, supporting the Team Managers to further develop the service and providing day-to-day consultation, direction, and support to the practitioners.

Key Responsibilities

  • Support the Team Managers to develop the transitions service
  • Provide day-to-day consultation, direction, and support to the practitioners
  • Carry a small caseload of challenging and complex cases
  • Lead by example to model best practice

Requirements

  • Qualified and registered Social Worker or Occupational Therapist
  • Minimum of 2 years post-qualifying experience for Social Workers
  • Previous experience working in the community or with young adults/transitions services for Occupational Therapists
  • Ability to travel across the City
