About the Role:

We are seeking a highly organized and communicative Outcome Broker to join our Integrated Community Discharge Team within Coventry City Council. The successful candidate will work closely with social workers, community case workers, and occupational therapists to source support for individuals with learning disabilities, mental health conditions, physical impairments, and older people.

Key Responsibilities:

  • Source support to effectively meet the needs of individuals with diverse needs and backgrounds
  • Work closely with a multidisciplinary team to provide comprehensive support services
  • Comply with relevant legislation, including the Care Act, Mental Capacity Act, and Mental Health Act
  • Promote asset-based social work and support individuals to contribute to their community

Requirements:

  • High level of organizational skills and ability to work effectively in a team environment
  • Excellent communication skills, including written skills, and ability to build strong relationships with stakeholders
  • Excellent interpersonal, negotiation, problem-solving, and analytical skills
  • Knowledge of computer programs, including spreadsheets and databases
