We are looking for a talented Support Practitioner - Dunoon & Cowal Role to join our team at Carr Gomm. As a key member of our team, you will provide immediate support in urgent situations, prioritising the well-being and safety of individuals within our community.
Key Responsibilities:- You will deliver personal care and practical support, taking into account the diverse needs and preferences of each individual.
- Through effective communication, you will carry out dynamic risk assessments and screen needs, identifying areas for improvement.
- To prevent hospital admissions, you will collaborate with partner organisations to develop comprehensive care plans.
