Support and Care Coordinator

Kisimul Group Ltd. is committed to delivering exceptional support services to young adults with complex needs. We are seeking a skilled and compassionate individual to fill the role of Support and Care Coordinator.
Key Responsibilities:
- Provide comprehensive support to safeguard young adults at all times in partnership with the staff team.
- Develop and implement personalized care plans to meet the unique needs of each young adult.
- Facilitate inclusive recreational activities that promote social interaction and skill development.
- Collaborate with healthcare professionals to ensure seamless care transitions.
- Maintain accurate records to inform evidence-based practice.
Requirements and Qualifications:
- A strong background in health and social care, preferably with experience working with vulnerable populations.
- Excellent communication and interpersonal skills to build rapport with young adults, families, and multidisciplinary teams.
- Ability to work independently and as part of a cohesive team.
- Strong computer literacy to efficiently manage electronic records and participate in online training sessions.
- Diploma Level 2 in Health & Social Care or equivalent qualification.
Salary Details:
The estimated annual salary for this role is £28,000 - £33,000, based on your level of experience and qualifications.
