About the Role

We are seeking an experienced and skilled Senior Integrated Discharge Practitioner to join our team at Greater Manchester Mental Health NHSFT. As a key member of our leadership team, you will play a vital role in ensuring the safe and timely discharge of patients from our inpatient units.

Key Responsibilities
  • Provide strong clinical leadership and line management to our Manchester teams, promoting a positive and therapeutic culture that empowers staff to achieve optimal performance.
  • Utilize your expertise to provide options and resolutions for complex cases, involving multidisciplinary teams, families, and carers.
  • Oversee the daily delivery of discharges, including complex cases and patients clinically ready for discharge.
  • Provide effective leadership for our Manchester Patient Flow Service spoke practitioners, promoting a positive and therapeutic culture.
  • Support the Care group at internal and external patient flow forums, sharing information, supporting improvement plans, and taking issues forward.
  • Participate in local and regional networks to promote Manchester care group and patient flow.
  • Retain daily oversight of discharge progress, providing live and accurate updates to the Head of operations, Associate directors, and Strategic lead as required.
Requirements
  • A social care qualification at degree level or equivalent experience.
  • Education to masters level or equivalent professional qualification or commitment to undertake.
  • Satisfactory record of continuous professional development.
  • Significant track record of employment at an operational level, delivering services in a mental health provider.
  • Evidence of leading service change with colleagues.
  • Experience of working in partnerships with other organizations to deliver services.
  • Experience as a practicing social care professional in a mental health environment.
  • Understanding of funding and panel processes across the organization for alternative provision to hospital beds.
  • High-level understanding of National Policy relating to funding for MH inpatient beds and discharge processes.
