Memory Nurse Specialist

Leeds and York Partnership NHS Foundation Trust is a leading provider of mental health and learning disability services in Leeds.
We are currently seeking an experienced Memory Assessment Nurse to join our team, working part-time hours of 30 per week. The successful applicant will be based in the East/North East locality, near Chapeltown in Leeds LS7.
This exciting opportunity is ideal for a committed and motivated nurse practitioner who wants to consolidate their practice in memory services and develop their skills and knowledge.
The role involves assessing service users for signs of organic progressive disease (dementia) and discussing findings with the multidisciplinary team to determine a potential diagnosis and supportive care plan.
As a Memory Assessment Nurse, you will offer post-diagnostic support, including practical information, signposting, and emotional support after diagnosis.
The successful candidate will have a full UK driving licence and access to a vehicle, as travel may be required for home visits and clinics.
Applicants should have relevant experience and knowledge of memory assessments and evidence-based tools used for diagnosis.
The ideal candidate will have excellent interpersonal skills and the ability to support service users and their families/carers practically and emotionally post-diagnosis.
The salary for this role is estimated to be £33,430 - £43,041 per annum, depending on qualifications and experience.
